Why hair is lost

Male pattern hair loss is driven by sensitivity of scalp follicles to dihydrotestosterone (DHT), a testosterone metabolite. Follicles miniaturise over years until they no longer produce visible hair. The pattern (receding hairline + crown) is genetic.

Other causes to rule out

  • Iron, vitamin D or thyroid deficiency.
  • Telogen effluvium after illness or stress.
  • Alopecia areata (patchy autoimmune loss).
  • Scarring alopecias — refer to dermatology.

Finasteride

Oral 1mg daily blocks DHT. Around 80–90% of men halt loss; a third see visible regrowth. Side effects include reduced libido or erectile difficulty in ~2%, usually reversible on stopping. Not for women of reproductive age.

Minoxidil

Topical 5% solution or foam applied twice daily. Works best at the crown. Mild scalp irritation is the commonest side effect. Combining with finasteride is more effective than either alone.

  • Dutasteride: stronger DHT blocker, off-label for hair.
  • PRP injections: mixed evidence.
  • Hair transplant: for established loss; ongoing finasteride still recommended.
