Key points

  • IBS affects roughly 1 in 5 UK adults — a real condition with effective management.
  • Diagnosis is clinical — based on symptom pattern after excluding red flags.
  • Low FODMAP diet helps many — best done with a dietitian.
  • Antispasmodics, peppermint oil, and (when needed) tricyclics or SSRIs help.
  • An online GP can assess, prescribe and refer.

Symptoms

  • Recurrent abdominal pain related to bowel habit.
  • Bloating.
  • Diarrhoea, constipation or alternating.
  • Mucus in stool.
  • Symptoms relieved by passing stool.

Diagnosis

Made clinically when symptoms fit the Rome IV criteria and no red flags are present. Routine tests: FBC, CRP, coeliac screen, faecal calprotectin (to exclude IBD).

Management

  • Low FODMAP diet (with a dietitian).
  • Soluble fibre (ispaghula).
  • Antispasmodics: mebeverine, peppermint oil.
  • Loperamide for diarrhoea-predominant.
  • Laxatives for constipation-predominant.
  • Low-dose tricyclic (amitriptyline) for pain.
  • SSRI for those with anxiety overlap.
  • Gut-directed CBT and hypnotherapy.
